Leaving hospital or struggling with mental illness

If you are facing homelessness on leaving hospital, or struggling with mental illness, there are a number of options available to you.

Rethink provides support and advice for people struggling with mental health issues. They can advise on everything from treatment and care to benefits and employment advice.

Leaving hospital Toggle accordion

If you are facing homelessness at the end of a stay in hospital, please get in touch with patient discharge team. They may put you in contact with the Housing Options team at your local council through a Duty To Refer.

On discharge from hospital, your hospital discharge team can put you in contact with a Community Navigator, who can help you access the local services you require to support your needs.
